ACCESS CENTER COORDINATOR Community Mental Health Center (Cicero)

Responsibilities include conducting mental health assessments, preparing initial service plans, making case assignments, providing phone support and office-based case management services to link clients to community services. Requires masters degree in social work, counseling or related field. Prefer candidate with professional license. Must have very good assessment, organizational, communication, writing and computer skills. Previous experience conducting assessments and providing case management services a plus. This is the key position within the agency that manages and coordinates clients access to services within the agency and the community.

REQUIRES FLUENCY IN ENGLISH AND SPANISH.
